Create your form (see Designing Your Planning Form)

Open an event or create a new event (see Creating an Event )

Add a Form To-Do to the event portal by clicking on the plus button.

Add title, description, and form to the To-Do. Select Preview To-Do

(Optional) Save the form as an automation by selecting Save To-Do as Portal Template
